COVID-19 Vaccination Programme Reservation(BioNTech ancestral strain/bivalent vaccine )

Terms & Conditions

• Advanced booking is required for the vaccination service.
• This service is eligible to:

  (1) Citizens belonging to the following priority groups, if they have completed the initial doses, they can receive an additional vaccine booster at least 180 days after their last dose or COVID-19 infection (whichever is later) free of charge in 2023, regardless of the number of vaccine doses they received in the past:

  – Individuals aged 50 or above (including elderly living in residential care homes)
  – Persons aged 18 to 49 years withunderlying comorbidities
  – Persons aged 6 months or above* and with immunocompromising conditions
  – Pregnant women
  – Healthcare workers

  (2) For those who aged 6 months or above* and not belonging to any of the aforementioned priority groups, they can continue to receive the first three doses of COVID-19 vaccine free of charge if they have never been infected with COVID-19.

  *We DO NOT provide BioNTech Vaccination to children aged 6 months to 11 years

  The Government sets up CCVCs for children aged 6 months to 11 to receive the BioNTech vaccine. Apart from the CCVCs, children aged below 12 cannot receive BioNTech vaccine at other venues. The CCVC at Hong Kong Children’s Hospital also provides vaccination service to children and adolescents aged 6 months to 17 years with the Sinovac vaccine.

• Your reservation could be cancelled if the recipient cannot show the relative documents or found that recipient are not eligible to vaccination.
• Your reservation could be cancelled if the booking number is under 6. We will contact you for the booking re-arrangement 24 hours prior to your appointment.
• If you are immunocompromised*, please bring the relevant medical certificate to the designated vaccination venue on the day of vaccination in order to confirm that you are eligible for the vaccination.
• * Immunocompromised persons include:
  1. Cancer or hematological malignancy on active immunosuppressive treatment now or in the past 12 months
  2. Recipients of solid organ transplant or stem cell transplant on immunosuppressive treatment
  3. Severe primary immunodeficiency or on chronic dialysis
  4. Advanced or untreated HIV disease
  5. On active immunosuppressive drugs or immunosuppressive chemotherapy/radiotherapy in past 6 months
• If you are aged 12 to 17, please download the consent for vaccination for completion by your parent/ guardian. On the day of vaccination, please bring the signed consent along and be accompanied by your parent / guardian upon arrival.
• If you have history of immediate (within 1 hour) severe allergic reaction to prior COVID-19 vaccination or to more than 1 class of drugs, or allergic reaction to prior COVID-19 vaccination which is not self-limiting or did not resolve by oral anti-allergy drugs, please do not make your appointment. You should consult your doctor for advice first.
• If you are doubtful about the suitability for vaccination due to your medical illnesses, please do not make your appointment. You should consult your family doctor for advice first.
• If you (or your carer(s)) want to know more information such as the suitability, efficacy, side effects and the potential adverse reactions of vaccination, please visit the website at www.covidvaccine.gov.hk.
• Other treatment/investigation costs are not included in this service.
• This service is non-transferable and non-refundable.
• Once the registration and vaccination schedule is confirmed, no rescheduling or cancellation will be accepted.
• In order to register online booking via Virtus, you will be required to provide Virtus with your name, gender, date of birth, Hong Kong Identity Card number, e-mail and local mobile phone number which can receive SMS. Your personal data will be used by Virtus for processing the booking and verifying eligibility for COVID-19 Vaccination Programme. Virtus will send you the appointment confirmation upon successful registration and reminder message to your provided local mobile phone number.
• Your personal data and COVID-19 vaccination related information will be stored as part of your medical records maintained by Virtus. It is not mandatory for you to provide the personal data as required, however, if the required personal data is not provided or that the personal data provided is inaccurate, incomplete, or false, Virtus will not be able to register you for the COVID-19 Vaccination Programme. Virtus reserves the right to review the registration, and where appropriate, reject or withdraw the registration if any information provided is invalid, inaccurate, incomplete, false or misleading.
• Government guidelines and recommendations may be subject to changes without notice and you should refer to the latest recommendations and information published by the Government before registering for the vaccine. Virtus shall not be responsible for any loss, damage, expenses or injury arising from your use of wrong information.
• I have read and understood all information as provided in the factsheet and the Statement of Purposes of Collection of Personal Data, and I consent to the administration of COVID-19 Vaccination to me / my child / my ward under the COVID-19 Vaccination Programme; and the Department of Health and the relevant organizations (collaborated with the Government ( including the University of Hong Kong ))’s access to and use of (i)my / my child / my ward’s personal data contained herein and (ii) my / my child / my ward’s clinical data held by the Hospital Authority and the relevant private healthcare facilities and healthcare professionals, for the purpose of continuously monitoring the safety and clinical events associated with COVID-19 Vaccination by the Department of Health insofar as such access and use are necessary for the purpose.
• Virtus Medical Group reserves the right to amend the above terms and conditions without prior notice. In case of any dispute, the decision of Virtus Medical Group shall be final.
